Please note: filling in an application does not guarantee approval, some dogs have more than one application and all applications are processed on their own merit. QSAR Inc will always approve on best fit for the dog and adoptive family not on a first in basis.

To complete this application you will need:

• A valid photo ID (License or Passport) with clear photo's of both the front and back (if license) or the ID page if its a passport.
• A copy of your landlords approval letter if you are renting (pdf preferably)
• At least one good image of your yard showing your fencing.

Please make sure you have the name of the dog you would like to adopt.
